Intra-hepatic Cholestasis of Pregnancy (ICP) is an uncommon condition in the first trimester and is associated with significant obstetric complications. We present the case of a 27-year-old, para 2 female who developed severe, early-onset ICP with persistently elevated bile acids (BAs), severe pruritus, and premature delivery at 31 weeks' gestation.
