Survey on Anaesthesia for Cervical Cerclage
Background
•A stitch placed around the cervix to reduce the risk of premature delivery
•No uniform anaestheticmanagement.
•Variable practice across the world.
•Results from a survey among the members of OAA - 348 responses
Special Considerations
•Chlorprocaine is a reasonable option,
•Abdominal cerclage is a laparoscopic procedure done at selected centresonly – Requires GA.
•Emergency / Salvage Cerclage - High risk of foetal loss – Consider lateral position for spinal or GA.
•Coughing is used to test the suture. High blocks can impair coughing.
Conclusion
•Spinal anaesthesia is preferred technique,
•Drug of choice
•Prilocaine 2% Heavy (2 – 3 ml)
•Bupivacaine 0.5% Heavy (2 – 2.4 ml)
•Opioids usually not needed,
•Desired block height - T10/T11,
•Individualize anaesthetictechnique.
