Use TRM techniques to improve the process from receipt of pathological tissue specimens to report completion
Introduction:
Patients' safety issues have always been the most important thing for medical staff. Regardless of the medical staff of patients, or medical staff who supported after post -defense, everyone puts the "patient safety" in their hearts.
In the pathological department, pathological tissue specimen is as important as patients. Therefore, use TRM techniques to improve the process from receipt of pathological tissue specimens to report completion.
Methods:
1. Establish a TRM team, arrange TRM education and training courses, and help team members understand TRM techniques.
2. All members worked together to review the "process from receipt of pathological tissue specimens to report completion" and drew a flow chart. Organize 14 steps and mark the people who perform the steps on the flow chart.
3. Set a target value, and monthly statistics "Completion rate of pathological tissue report within five days" is >93%.
Results:
After brainstorming among the team members, the team members selected three topics that need to be improved by introducing TRM. The three topics are (1.) The pathological tissue specimens are different from pathological tissue order, (2.) The pathological samples of difficult cases are not known. How to carry out the cutting process, (3.) The number of tissue specimen embedding cassettes does not match the number of HE stained slides.
From January to December 2023, "Pathology report completion rate within five days". They are: 97.09%, 96.25%, 95.49%, 97.58%, 97.73%, 98.48%, 97.83%, 97.24%, 97.88%, 97.05%, 97.4%, 97.73%, all reaching the target value >93%. Among them, the best performance in June was 98.48%, and the achievement rate was as high as 105%. The worst performance in March was 95.49%, and the achievement rate was also as high as 102%.
