Introduction: The 2017 Taiwan National Health Interview Survey revealed that over 60% of the elderly have at least two chronic diseases. Early intervention can prevent or delay chronic diseases, making education and training essential for improving primary care providers' ability to handle common risk factors.
Methods: During the prep period, through expert group and focus group, experts in various fields and clinical practice opinions were collected, and the core competencies required for chronic disease interdisciplinary integrated care case management were summarized by references, and then competency-oriented through the nominal group technique, gradually complete the training syllabus. According to the course learning objectives and key points of the course, complete the production of teaching materials and test questions, and record digital courses and make case discussion workshop teaching materials. The structure of the course covers clinical caregivers' ability to manage chronic disease risk factors and core skills (including knowledge of health behavior change, and motivational interviews).
From July 1, 2022 to Nov 16, digital courses and 4 case discussion workshops were piloted, and a total of 115 students were trained, including doctors, nurses, nutritionists, and pharmacists. The course modules were revised based on the feedback from the trainees, and put forward suggestions for the application of the training program, as a reference for the future training of various academic associations and medical institutions.
Analysis method: use paired sample t-test to analyze the difference in test scores of students before and after the digital course.
Results: Digital Course Satisfaction: The digital course achieved an average satisfaction rating of 89%, while the workshops had an average rating of 96%. Trainees found the course content diversified, meeting their needs and applicable to nursing cases. It also assisted in clarifying case-related questions.
Test Results: The pre-class test had a mean score of 74.96 points, increasing to 84.43 points in the post-class test. The mean score difference was 9.47 points, with a statistically significant improvement (t = -4.99, p = 0.000, p < 0.05). This pilot program resulted in a noticeable performance improvement for participating students.
Conclusion: This training program enhances primary care providers' abilities to prevent and treat common risk factors of chronic diseases early on. It also empowers medical staff to guide patients in self-health management and behavior change, leading to effective chronic disease prevention and reduced follow-up medical resource burden. However, promoting this program nationwide and encouraging more medical staff to participate will necessitate the development of relevant supporting measures.
