Right Axillary Artery Cannulation: A Standardized Approach in Minimally Invasive Coronary Surgery
Objective:
This video demonstrates the standardized technique of direct right axillary artery (RAA) cannulation for antegrade perfusion during minimally invasive coronary artery bypass grafting (CABG). It highlights the advantages of this approach in patients with significant atherosclerosis, where retrograde femoral artery cannulation is contraindicated.
Methods:
Between November 2019 and December 2023, 413 patients underwent CABG via left anterior minithoracotomy with cardiopulmonary bypass using RAA cannulation. Outcomes assessed included intraoperative complications, postoperative neurological events, and long-term cannulation-related issues.
Results:
RAA cannulation was successful in 100% of cases, with minimal intraoperative complications and a postoperative stroke rate of 0.5%. Follow-up revealed no significant blood pressure discrepancies, permanent neurological deficits, or need for re-interventions.
Conclusions:
RAA cannulation provides a safe and effective strategy for antegrade perfusion in minimally invasive CABG, achieving excellent outcomes with minimal complications. This technique expands the applicability of minimally invasive approaches to a broader patient population.
