Case Presentation
Zygomatic implants have been associated with favorable long-term outcomes in the rehabilitation of severely atrophic maxillae. Several benefits have been reported, including reduced invasiveness and morbidity, less discomfort for the patient, and shortened rehabilitation times when compared to alternatives such as staged grafting. Complications and failures, however, are observed both before and after loading. These can include sinusitis, oro-antral communications and periimplantitis. There are reports focusing on management of complications, and subsequent surgical and prosthetic approaches. This patient report describes failure leading to the removal of implants (zygomatic and conventional), and retreatment with zygomatic implants used according to the ‘Quad Zygomatic ZAGA’ protocol. Quad Zygomatic implants were placed with a template according to the ‘Quad Zygomatic ZAGA’ protocol. Treatment included utilization of a buccal fat pad flap to close an oral-antral communication secondary to failure of previous zygomatic implants. The newly positioned zygomatic implants were restored with an implant-supported overdenture utilizing a CAD/CAM fabricated bar. The patient has not encountered any complications at the 8 month follow up. Patient rehabilitation incorporating zygomatic implants, and a CAD-CAM fabricated bar supporting an overdenture, can be a predictable treatment option for severely atrophic maxillae secondary to complications and failure of previous procedures.
