Comparative Analysis of Reduction Mammaplasty vs. Oncoplastic Breast Reconstruction: Assessing Clinical, Aesthetic, and Patient-Reported Outcomes
Introduction:
•Oncoplastic breast reconstruction benefits women undergoing breast conservation therapy (BCT) by alleviating macromastia symptoms and enhancing symmetry, shape, and cosmesis.
•Compared to BCT alone:
1.With Increased OR time
2.Additional surgical site
3.Location of tumor limits pedicle choice
•No prior investigation on (a) post-operative clinical outcomes, (b) patient reported quality-of-life, and (c) aesthetic results of oncoplastic reconstruction (OP) compared to standard bilateral breast reduction (BBR).
Methods:
•Single-surgeon, single-institution retrospective chart review from 4/2022-12/2023.
•Data collection on:
(a) Patient demographics and post-op clinical data per EMR review
(b) Patient-reported QoL outcomes: pre- vs. post-op BREAST-Q survey results
(c) Aesthetic outcomes: Ten 1:1 propensity score matched cases
Results:
•92 total patients, 56 (60.9%) received BBR and 36 (39.1%) oncoplastic reconstruction.
•BBR cohort significantly younger and with >resection weight.
•OP cohort with higher BMI and ASA class.
Clinical Outcomes:
•OP and BBR cohorts with comparable post-op clinical complication profile.
QoL Outcomes:
•Both cohorts with significant improvements in all BREAST-Q pairings (psychosocial, sexual, physical well-being and breast satisfaction), through greater increase in BBR group
Aesthetic Outcomes:
Conclusion:
•Notable differences in demographics and patient characteristics (age, ASA, race)
•Both groups with significant QoL improvement
•Comparable clinical and aesthetic results between the two cohorts
•Oncoplastic breast reconstruction be more readily discussed as a favorable surgical option when lumpectomy is indicated
