Abstract

Background: Alzheimer’s disease (AD) is a leading cause of neurodegeneration globally, with increasing incidence in Taiwan. While Aβ plaques and neurofibrillary tangles are the hallmark pathologies, recent evidence suggests that metabolic dysregulation, including lipid profile changes, plays a critical role in AD progression. This study investigates the predictive value of serum metabolic indices such as triglyceride-glucose (TyG) index, triglycerides (TG)/high-density lipoprotein-cholesterol (HDL) ratio, low-density lipoprotein-cholesterol (LDL)/HDL ratio, and total cholesterol (TC)/HDL ratio in identifying AD risk in the Taiwanese population.

Methods: A cross-sectional study involving 73 AD patients and 60 controls was conducted using data from Taipei Medical University Joint Biobank. Clinical chemistry data, including alanine transaminase, creatinine, fasting glucose, glycohemoglobin A1c, HDL, LDL, TC, TG, and total protein, were analyzed. AD diagnosis was based on DSM-IV criteria and an MMSE score below 26.

Results: Receiver operating characteristic curve analysis showed poor predictive ability for the TyG index (AUC = 0.536), with performance close to random chance. The TG/HDL ratio exhibited a modest predictive ability (AUC = 0.663), with a 28% increase in AD odds per unit increase. The LDL/HDL ratio demonstrated fair predictive accuracy (AUC = 0.737), associated with a more than twofold increase in AD risk. The TC/HDL ratio showed similar fair predictive ability (AUC = 0.740), with a 104% increase in AD odds per unit increase.

Conclusion: TG/HDL, LDL/HDL, and TC/HDL ratios are significantly associated with AD risk, suggesting their potential use as non-invasive biomarkers for early detection. The TyG index, however, showed limited utility in predicting AD. Further research is needed to validate these findings and explore population-specific factors influencing the associations between lipid metabolism and AD risk.
