Total mesorectal excision (TME) remains the standard for rectal cancer surgery. Laparoscopic TME may be technically challenging in narrow pelvises or distal tumors. Transanal TME (taTME) provides direct distal access to the rectum and mesorectum, potentially improving visualization, sphincter preservation, and oncological precision.
