Background

•Increased rates of difficult intubation, including failed intubation, airway complications and awareness1
•
•Checklists and system-based approaches can facilitate improved outcomes by removing mental load from clinicians
•
•We have implemented a quality improvement bundle alongside an RSI checklist recommending:
qRamped position
qBIS
qLow flow nasal oxygen (LFNO)

Videolaryngoscopy (VL) use

Methods

•Data collected on all Obstetric GAs  from January 2023 to September 2024
•
•Data was recorded by anaesthetists involved in the case for ramped position, BIS, LFNO, use of VL and successful intubations
•
•Local Clinical Governance team and Caldicott approval was granted

Results

•Total of 548 Obstetric GA cases during study period
•
•Intubation data available for 473/548 (86.3%) cases
•
•All measured outcomes were >91%
•
Successful 1st pass intubation was achieved in 96.2% of cases

Discussion

•Obstetric GAs can be stressful and challenging – often performed by trainees out of hours
•
•A consistent, systems-based approach can ensure that patients are kept safe
•
•We show that with the use of a QI bundle alongside an RSI checklist maintains a high rate of successful first pass intubations, with few failed or difficult intubations
