Reduction on the discrepancy rate of medication reconciliation for scheduled inpatient operation among the elderly in regional hospital of southern Taiwan
Introduction: Most of the scheduled in-hospital operations among elderly patients are arranged by urologists (42.4%) in our hospital. Medication errors are a common occurrence in surgeries and the management of home medications is strongly linked to these errors. The primary cause of medication errors is medication discrepancy, which include misuse, underuse, and overuse. Studies have shown that many of the adverse events caused by medication are preventable. Furthermore, polypharmacy is a common scenario among the elderly population. Therefore, reducing the medication discrepancy rate between hospital and home medication prescriptions for elderly inpatients is the primary goal of this research.
