Introduction:
Medication errors constitute a major concern in different age groups; children are distinctively prone to this problem.
From January 2020 to June 2022, there were 15 medication errors in the Department of Pediatrics.
Aim
To eliminate the incidence of medication errors by physicians in the Department of Pediatrics by the end of September 2022.
Methods:
From August to September 2022, the following alternative solutions were implemented:
A System of Review for Medication Process Implementation
Series of Workshops/ Lectures on Drug Computation, Prescriptions, Ordering, and Administering Medications
Provision of a List of available drug preparation in the pharmacy and other references (MIMS)
Creation of Information, Education, and Communication (IEC) materials on medication ordering/ prescribing
Involvement of Patient/caregiver through Patient Medication-Education-Checklist
Results
90 patient charts were reviewed for medication errors
100% of ward residents attended the medication workshops
100% of areas were issued copies of the drug list, (MIMS), and IEC materials
100% of patients receiving medications from piloted areas utilized the Patient- Medication- Education-Checklist
Zero medication error
Conclusion
All implemented alternative solutions for preventing medication errors were effective.
Future studies on implementing alternative solutions in other clinical areas are recommended.
