BACKGROUND：This video aims to investigate the possibility of redo aortic and mitral valve replacement with reconstruction of the intervalvular fibrous body ("Chimney" Commando procedure) through right anterior thoracotomy (3rd intercostal space) approach.

METHODS：A 70-year-old male patient(body weight 71kg; Height 176cm) with endocarditis of both aortic and mitral bioprostheses implanted about 10 years ago was operated with "Chimney" Commando procedure(29mm mitral bioprosthesis with Dacron skirt and 25mm aortic bioprosthesis) by right jugular vein, femoral vein and artery cannulation to establish CPB.

RESULTS: CPB time was 160 minutes and aortic clamp time 129 minutes. Postoperative recovery was uneventful.

CONCLUSIONS: Redo Commando procedure through right anterior thoracotomy can be safely performed with good results in selected patients.
