INTRODUCTION
Background: According to the guidelines, in centres with a high degree of procedural expertise, mitral valve repair is the treatment of choice. Longer life expectancy leads to an increase in cardiac surgery after the age of 80.
Our purpose: The purpose of this study is to summarise the experience of endoscopic mitral valve surgery in octogenarians, evaluating the safety of the procedures and the short- and medium-term results in terms of life expectancy of life.
METHODS
From July 2015 to April 2023 35 patients with age of 80 or aboveunderwent isolated or concomitantmitro-tricuspid surgery via 4 cm right thoracotomy in totallyendoscopic setup. All electronichealth records were reviewed for this study and all data are available in our Institute'selectronic health system. Telephone calls were used for recent follow-up data and recent outpatient clinic records.
RESULTS
The mean age of the patients was82.7 years, 62% were male. Mitralvalve repair surgery wasperformed in 76% of the casesexamined. The average hospital stay at our Institute was 8.3 days. At a mean follow-up of 3 years, the survival rate was 88.2%. Only2 patients among the survivorshad moderate or greater residualmitral valve regurgitation. The mean NYHA class was 1.4 (onlythree patients had NYHA class 3 or higher).
CONCLUSIONS
Endoscopic mitral valve surgery is a valid technique for special groups such asoctogenarians. It can produce excellent resultswith a good repair rate and less surgical trauma. Ourexperience showed safe feasibility, excellent earlyand good mid-term results.
