Aqueductal stenosis causing non-communicating hydrocephalus presents significant anesthetic challenges in pregnancy due to the risk of CSF pressure shifts with neuraxial techniques. This case demonstrates the successful use of general anesthesia with adjunct quadratus lumborum blocks to provide safe, opioid-sparing analgesia for elective cesarean delivery in a high-risk patient.
