Pregnancy Luteoma: The Masquerading Ovarian Enigma – A Case Report
Authors: Dr. Gaurang Narayan, Dr. Sachin Wankhede, Dr. Prachi C. Thool
This case report details a rare benign ovarian lesion known as pregnancy luteoma, which mimics ovarian malignancy but is hormonally induced and typically regresses postpartum. The case involves a 27-year-old primigravida who presented with abdominal distension, massive ascites, and a suspicious adnexal mass. Elevated CA-125 and imaging findings suggested a malignancy, prompting a right salpingo-oophorectomy.
Histopathology confirmed the mass as a luteoma. The patient later delivered a healthy baby via cesarean section. This case emphasizes:
-
The diagnostic dilemma posed by luteomas due to their clinical and radiological resemblance to malignancy.
-
The importance of cautious management to avoid unnecessary surgery.
-
Luteomas should be considered in the differential diagnosis of adnexal masses during pregnancy, especially when accompanied by ascites and elevated tumor markers.
