Accuracy of self-reported gestation estimation in women presenting for abortion, NHS Fife

Dr Carolyn Ford, consultant O&G; Dr Emily Hennah, clinical fellow, both NHS Fife Gynaecology

Background: Telemedicine - Use of technology; Increases access; Employs triage system; Generally safe and effective

Staff sometimes question whether women will accurately self-report gestation.

Study: Our region looked  retrospectively at the accuracy of self-reported gestation in women presenting for abortion.

We compared LMP reported on referral with scan gestation.

Results: 112 women self-referred in 1 month.

On referral:

71% gave LMP to nearest day/week

24% gave to week/fortnight

5% uncertain of LMP

On scan:

5% not pregnant

90% <12/40

5% >12/40

Accuracy of estimation (not pregnant excluded):

77% accurate to nearest week and further 8% accurate to nearest 2 weeks (total 85%).

5% had not known LMP – all <12/40

10% underestimated gestation – median 18 days.

Conclusion: The majority of women are able to estimate their gestation to the nearest week.
Most present in the first trimester.
Many of those who cannot estimate their gestation would be picked up by triage system.
