Title :Multidisciplinary Fetal Monitoring Training:  Is your local training achieving the objectives?

Objective: To share our experience and raise the importance of obtaining regular structured feedback from multidisciplinary maternity staff about fetal monitoring training alongside auditing clinical outcomes to determine if training is effective.

Design: Our feedback was completed by all staff at the end of each training day using an online questionnaire. Training was delivered in person by a multidisciplinary faculty to groups averaging 20-30 multidisciplinary staff.

Our training was designed in line with national report recommendations, including Saving Babies Lives Care Bundle version 2, HSIB, Each Baby Counts and Ockenden. Feedback included rating for each area of training, including human factors, situational awareness, effective clinical escalation, fetal physiology, cCTG, evidence and risk assessment, intelligent intermittent auscultation, learning from incidents and themes locally and nationally and equipment training. We included specific questions addressing knowledge and nontechnical team working skills. Questions were “Do you feel participation in the study day has improved your knowledge and competence in CTG interpretation?” The same question was asked for cCTG and IIA. Participants were also asked “Do you feel confident escalating concerns/ issues in clinical practice?” And “How could we enhance fetal monitoring training moving forward?” These questions were specifically asked to identify areas of concern/ for development within our maternity unit that are not determinable from auditing outcomes, and to improve effectiveness of the study day.

Results: Our response rate was 82%. It took staff an average of 2 minutes to complete the questionnaire. 91% of candidates responded that their knowledge and competence is improved as a result of the study day. 90% of candidates felt confident in escalating concerns and qualitative responses were collected from those who did not. Interestingly the feedback highlighted that 75% of attendees preferred training in person rather than online. Staff valued all topics covered on the day and greatly valued training as a multidisciplinary group.

Discussion: Various national reports regarding maternity care across UK have consistently highlighted the importance of fetal monitoring training with expanded national recommendations for the delivery, duration and content of the training.

The quality of the training still remains varied across UK Trusts and there is very little guidance on obtaining structured feedback .We call for national standardised structured feedback recommendations for all Trusts to ensure quality training is being delivered and unified objectives are being achieved nationally.
