Expanding the Reconstructive Armamentarium: Use of MTF Profile® Costal Cartilage Allograft for Helical Rim Reconstruction in a Pediatric Patient

Purpose

Auricular reconstruction after traumatic helical rim loss is challenging, particularly in pediatric patients with limited donor sites. Autologous costal cartilage remains the gold standard but carries donor-site morbidity and contour variability. MTF Extearna®, designed for total ear reconstruction, may be excessive for partial defects. We report the first known application of MTF Profile® Costal Cartilage Allograft—commonly used in revision rhinoplasty—for partial auricular reconstruction, providing structural integrity and versatility for targeted framework restoration.

Methods

A 13-year-old male with a remote dog bite injury presented with loss of two-thirds of the right helical rim and 1.5 × 1.5 cm scapha defect. Following scar excision and exposure of native cartilage, the MTF Profile® was sculpted to recreate the helical rim and scapha. Grafts were secured with 5-0 nylon sutures, and a 2 × 6 cm bipedicle mastoid skin flap provided soft-tissue coverage.

Results

The MTF Profile® demonstrated excellent handling and adaptability. Multiple configurations allowed selection of the optical piece while minimizing trimming and graft waste. Its preformed geometry and flexible rigidity enabled precise shaping of the complex auricular geometry, providing immediate structural restoration. These properties suggest a broader role for Profile cartilage as a low-waste, adaptable alternative in partial ear reconstruction.

Conclusions

MTF Profile® Costal Cartilage Allograft represents a novel, structurally robust option for segmental auricular reconstruction. Traditionally used in revision rhinoplasty, this is its first reported application for partial auricular framework reconstruction, offering a versatile, efficient alternative to previously described techniques.
