Early Clinical Outcomes And Amyloid-related Imaging Abnormalities In Lecanemab-treated Patients At An Academic Health System

Part of Topic

In a real-world clinical setting at an academic health system, lecanemab-treated patients generally experienced a cognitive and functional decline, as expected from the phase 3 trial. Infusion-related reaction rates were higher than observed in the phase 3 trial. ARIA-E rates were generally similar to what was reported in the clinical trial, but the ARIA-H rates were generally higher.

INTRODUCTION

Lecanemab is an anti-amyloid therapy for early symptomatic Alzheimer’s disease, with side effect of Infusion-related reactions (IRR) and amyloid-related imaging abnormalities (ARIA).1

1van Dyck et al., NEJM, 2023

METHODS

•Design: IRB-approved retrospective cohort study
•Participants: Lecanemab-treated patients with at least 6 months of follow-up (September 2023-February 2026)
•Data collection: Artificial-intelligence-assisted extraction of electronic health record data
•Primary outcomes: MMSE, FAQ, and QDRS trajectories modeled using linear mixed-effects models
•Secondary outcomes: IRR and ARIA rates

CONCLUSIONS

•The majority of lecanemab-treated patients experience a cognitive and functional decline on treatment, but clinical effectiveness cannot be inferred from these data
•IRR were generally mild, though the rates were generally higher than expected, possibly due to increased vigilance
•ARIA-E rates were generally as expected
•ARIA-H rates were generally higher, possibly due to use of more sensitive SWI sequences for monitoring

 FUTURE DIRECTIONS

•Comparison of lecanemab-treated patients with historical controls as an estimation of clinical effectiveness in a real-world setting
•Evaluation of donanemab outcomes
•Correlation of imaging outcomes and cognitive outcomes
