Neuropsychiatric symptoms are common in Alzheimer's disease and are associated with poorer clinical outcomes and increased caregiver burden. We developed and evaluated a clinical prediction model using routinely collected outpatient clinical data from 88 patients with Alzheimer's disease attending a public hospital in Peru. High neuropsychiatric symptom burden was defined using the Neuropsychiatric Inventory (NPI). The model included age, years of education, Clinical Dementia Rating, Blessed Dementia Scale, and disease duration. Longer disease duration was independently associated with high neuropsychiatric symptom burden. The model demonstrated good discrimination (AUC 0.76) and good calibration (Hosmer-Lemeshow p = 0.81). This simple prediction model may support early identification of high-risk patients and facilitate referral to specialized neurology or psychiatry services in resource-limited healthcare settings.
