This systematic review (94 studies, PROSPERO CRD42025064231) summarizes the evidence on blood-based biomarkers (pTau, GFAP, NfL, Aβ, tTau) in individuals with genetic intellectual disabilities (GenID), evaluating their diagnostic and prognostic value for dementia. pTau217, GFAP, pTau181, and NfL showed strong diagnostic and prognostic accuracy in Down syndrome (DS), while evidence in non-DS GenID remains limited.
