In patients with DLB, neflamapimod treatment increased right basal forebrain (BF) volume relative to placebo over the 16 weeks of the randomized period of a phase 2n cliinicala trial in patient with dementia with Lewy bodies (DLB), an effect that was maintained during an additional 32 weeks of open label treatment. In addition, during the open label extension, 32 weeks treatment increased functional connectivity between right BF and the right dorsal mode network (DMN)
Together with prior findings in early AD, these results support a beneficial effect of neflamapimod on basal forebrain atrophy, consistent with its proposed mechanism and preclinical evidence. The left-sided treatment effect is consistent with the greater vulnerability of the left basal forebrain in DLB (Churchill, 2026; Kanek, 2021).That right basal forebrain volume remained stable over 48 weeks with continuous treatment and stabilized after neflamapimod initiation in prior placebo recipients suggests that neflamapimod may slow progression of basal forebrain atrophy.
