Transforming Dementia Care With Digital Health: Redefining Access, Equity, And Continuity

Part of Topic

Title: Transforming Dementia Care Through Digital and Mobile Health: Redefining

Access, Equity, and Continuity Across Alzheimer’s Disease and Related Dementias

Theme: Dementia Care and Psychosocial Factors

Sub-theme: Dementia Care and Health Services Research

Topic Area: Digital and Mobile Health (Telemedicine, Virtual Care)

Presenting Author:

Background

The accelerating global burden of Alzheimer’s disease, mild cognitive impairment (MCI), and

related dementias has revealed significant limitations in traditional clinic-centered care models.

Workforce shortages, geographic isolation, mobility constraints, and socioeconomic inequities

continue to delay diagnosis, disrupt care continuity, and disproportionately affect underserved

populations. Digital and mobile health innovations, including telemedicine, virtual cognitive

assessment, remote symptom monitoring, and caregiver support platforms offer promising

opportunities to transform dementia care delivery; however, their real-world impact on care

quality, psychosocial outcomes, and health system performance remains insufficiently

synthesized

Purpose

To evaluate the effectiveness of digital and mobile health interventions in improving access,

continuity, equity, and psychosocial outcomes in the care of individuals living with Alzheimer’s

disease, MCI, and related dementias.

Methods

A structured integrative review of 100 peer-reviewed studies published between 2019 and 2025

was conducted using PubMed, Scopus, PsycINFO, and CINAHL. Studies examined

telemedicine-based dementia care, mobile health applications, remote monitoring technologies,

or virtual care models. Outcomes were synthesized across domains of clinical effectiveness,

caregiver burden, patient engagement, health service utilization, and health equity.

Results

These interventions consistently demonstrated improved access to specialized dementia care,

enhanced continuity of follow-up, and significant reductions in caregiver burden. Telemedicine-

based dementia care models achieved clinical outcomes comparable to in-person care while

substantially reducing missed appointments, travel burden, and care delays. Remote monitoringand mobile platforms improved detection and management of neuropsychiatric symptoms and

supported proactive care coordination, with particularly strong benefits observed in rural, low-

income, and mobility-limited populations.

Conclusions

Digital and mobile health technologies represent a transformative force in dementia care

delivery. Strategic integration into health systems can expand access, reduce inequities,

strengthen caregiver support, and enhance the sustainability of dementia care models.
