Abstract

Transforming Endometriosis Care: 18-Month Results of Robotic-Assisted Surgery at Mid Yorkshire Teaching NHS Trust

Mr. Alexander Oboh. Dr Muna Ewadh

Introduction

The Mid Yorkshire Teaching NHS Trust Endometriosis Centre, established in 2014, is a leading institution for advanced endometriosis management in Yorkshire. In August 2023, the centre adopted Robotic-Assisted Surgery (RAS) using the Da Vinci Surgical System for the treatment of severe Deep Infiltrating Endometriosis (DIE). This study presents the outcomes from the first 18 months of clinical experience involving a single consultant.

Method:

This is a retrospective study of all patients who had RAS for treatment of endometriosis between August 2023 and January 2025.

Results:

33 patients underwent RAS for endometriosis. The median patient age was 35–40 years, with BMI ranging from 19 to 39. Procedures included RAS excision of endometriosis (n=3), RAS excision of DIE and ovarian endometrioma (n=18), and RAS total laparoscopic hysterectomy with DIE excision (n=12). Three cases were performed in collaboration with a colorectal surgeon, involving bowel endometriosis shaving (n=2) and segmental bowel resection (n=1). Most cases (32/33) were completed without intraoperative complications. One patient sustained a ureteric injury during DIE nodule excision, which was successfully repaired robotically by a urologist with good postoperative outcome. The median hospital length of stay was one day (range: same-day discharge to four days). No readmissions were recorded within 30 days.

Conclusion:

These results highlight the safety and efficacy of RAS for treating severe endometriosis, including DIE at Mid Yorkshire endometriosis centre. The short recovery time and low complication rates underscore the potential of robotic surgery to enhance outcomes in complex cases.
