Effect of Aflibercept 2mg and Faricimab 6mg on the height of Retinal Pigment Epithelial Detachment in Neovascular AMD during the loading dose phase

Introduction :This observational study was to compare the effects of Aflibercept and Faricimab on the height of be retinal pigment epithelial detachment (RPED) in Polypoidal choroidal vasculopathy and in Retinal angiomatous proliferation lesions in patients with neovascular age-related macular degeneration.
Methods : In total 66 eyes of 60 patients were taken into consideration and were retrospectively analysed using multimodal imaging. Of these, 46 eyes were treated with intravitreal Aflibercept 2 mg injections and 20 eyes were treated with intravitreal Faricimab with 3 injections at 4 weekly intervals as loading dose . Measurements were taken before treatment and at 4 weeks , 8 weeks and at 12 weeks after the first treatment. The maximum height of the retinal pigment epithelial detachment were measured using optical coherence tomography. A paired t –test was used to compare the mean value of the RPED from baseline to the completion of loading dose. p-values below 0.05 were considered significant.
Results : In the Aflibercept group , the the maximum height of mean of 240 μm at baseline (215 ± 177 ) was reduced to 155 μm ± 62 μm at 4 weeks, 120 μm ± 48 μm after 8 weeks and 105 μm ± 42 μm after 12 weeks. In the Faricimab treated group , the maximum height of mean of 248 μm at baseline (248 ± 160) before treatment was reduced to 160 μm ± 64 μm at 4 weeks , 126 μm ± 50 μm at 8 weeks and 107 μm ±40 μm at 12 weeks. The mean change in the height from the baseline to 12 weeks in Aflibercept 2 mg was 116 μm ± 53 μm and in Faricimab group, it was 112 μm ± 50 μm . There weren’t any complications in either group.
Conclusion : 1. Both the groups showed statistically significant decrease in RPED height from baseline to
12 weeks (P < 0.001 in both groups).
2. The difference in mean changes of the height of RPED from baseline to 12 weeks did not
show any statistically significant difference between Aflibercept and Faricimab groups
( p value > 0.05)
