Critical Care - Neuroanesthesia
Propofol, known for its rapid onset and smooth recovery, is commonly used for induction and maintenance of anesthesia. Although generally safe, rare adverse reactions may occur. Our case describes a 24 year old male who experienced delayed unresponsiveness with irregular myoclonic movements following propofol. His clinical course did not align with malignant hyperthermia, propofol infusion syndrome, or seizures. Findings, including elevated CK and prolactin, support an idiosyncratic propofol reaction, potentially related to mitochondrial dysfunction or glycine receptor antagonism. This case highlights the need for increased awareness and further documentation of atypical responses to propofol to clarify underlying mechanisms and guide individualized anesthetic care.
