Silent machine alerts highlight a critical safety gap: anesthesia technicians play an essential role in detecting equipment failures, yet are often excluded from crisis training. An in-situ OR simulation of a silent “cooling fan failure” showed that their expertise is frequently overlooked, even though physicians rely on them to raise critical concerns. These findings emphasize the need for dedicated training that strengthens technicians’ leadership and speaking-up skills.
