Introduction

Tooth loss in the anterior maxilla can lead to rapid physiologic and

dimensional changes of both soft and hard tissues. Immediate implant

placement and provisionalization, the SameDay Concept, has become an

increasingly favoured approach in the esthetic zone as it preserves peri-

implant soft-tissues by maintaining the facial contour, papilla height,

gingival margin position and overall esthetic profile through site-specific

implant placement.

 

Methods

A 37-year-old female patient presented to the clinic with a remaining root

#11. Atraumatic extraction was performed followed by osteotomy

preparation on the palatal wall of the alveolar socket with the PROVATA®

surgical kit. A 4 x 15 mm Southern PROVATA® Co-Axis® 12° implant was

then placed with 50 Ncm final seating torque. The buccal gap was grafted

with 0.5 cc Puros® cortico-cancellous allograft. A temporary titanium

cylinder was seated and verified radiographically. A provisional crown was

selected from the 3M™ ION™ kit, luted intraorally to the temporary

cylinder, removed, and polished prior to insertion. Occlusion was adjusted

to avoid both centric and eccentric contacts.

Results and Conclusion

Atraumatic extraction with immediate site-specific implant placement and

provisionalization achieved peri-implant soft-tissue maintenance and

esthetic rehabilitation.
