Possibilities of the Off-Pump Autoarterial Myocardial Revascularization Using Two Internal Mammary Arteries

Purpose: to present and analyze our results of the OPCAB

 using only two internal mammary arteries.

Methods: Total number of 267 patients underwent off-pump

bilateral mammary artery coronary bypass grafting.

Surgical Technique:

Using skeletonized ITA (100%)
Trying to use ITA in situ position
Applying sequential BITA bypass grafting
Primary using LIMA and RIMA for LCA branches
Using arterial grafts for coronary arteries bypass with stenoses more than 70 %

Conclusion:Off-pump BIMA multiple CABG is a modern

 andsafemethod of  myocardialrevascularization.

It does not increasethesurgicalrisk in perioperative

period. Today itshouldbeconsideredasmethodofchoice in themajorityofpatientswithcoronaryarterydisease

undergoingmyocardialrevascularization.
