FOCAL  LASER ABLATION (FLA) OF PROSTATE CANCER   PRELIMINARY FUNCTIONAL RESULTS

BACKGROUND

If  you can see prostatic cancer using fusion MRI and can put a needle in the spot to biopsy it, why not stick a laser fiber in the tumor the same way to kill it?

RESULTS

From February   to October  2019 13 patients were included  with mean age 67 years

8 Tumors at low and 3 at intermediate risk

10 Monofocal lesions and 3 bifocal

Single lesion under 15 mm, double lesions under 6-7mm each

1 patient removed the catheter 48 hours after the procedure

No patient had urinary incontinence

No patient had sexual dysfunction

No patient had de novo LUTS

No patient had a biochemical relapse

CONCLUSION

•FLA provided excellent functional results, better than radical treatments ( Surgery or Radiotherapy) and other Focal therapies
•In low tumor risk and in well selected Patients FLA is proposed as an active treatment-bridge between active surveillance and radical treatment.
•In intermediate tumor risk FLA is suggested for pts  who want to maintain sexual activity and continence or old pts or those with comorbidities.
•Patients have to be very well informed of the innovative nature of FLA.It provides excellent results but cannot yet be compared with radical treatment in terms of oncological control because of limited data and short follow up
