Broader Neuromodulatory Effects From Common Peroneal Nerve Stimulation for Neuropathic Foot Pain
•Peripheral nerve stimulation (PNS) is an emerging, minimally invasive treatment for mononeuropathies and focal neuropathic pain.
•However, its role in the management of diffuse or bilateral neuropathies remains limited, primarily due to challenges in selectively targeting widespread symptoms with a small number of leads.
Case Description
•A 61-year-old woman presented with chronic and severe bilateral neuropathic foot pain.
•She was unable to tolerate multiple classes of systemic neuropathic pain medications and declined opioid therapy. Psychological interventions and physical therapy provided minimal benefits.
•Bilateral percutaneous PNS leads were implanted, targeting only the common peroneal nerves. Insurance limitations permitted the implantation of only two leads (Figure 1).
•Unexpectedly, she experienced near-complete resolution of bilateral foot pain shortly after stimulation began.
•Her pain remained well-controlled without the need for analgesics. At her three-year follow-up she reported sustained 60% pain relief.
