INTRAGASTRIC BALLOON OF 6 MONTHS WITH LOSS GREATER THAN 30% OF TOTAL BODY WEIGHT - TWO CASES REPORT

—The intragastric balloon (IGB) is an option for the treatment of obesity, a silicone device filled with saline and methylene blue and implanted through digestive endoscopy. Its goal is to promote a feeling of early satiety, reducing food consumption and facilitating a reeducation of the lifestyle. The duration of the balloon used in the reported cases is 6 months and the expected average loss is around 13% of the body weight loss (BWL%). Two cases of BIG use with a loss of more than 30% of total body weight are reported.
—Case 1: MPZ female patient, 38 years old, with initial weight of 100 kg and body mass index (BMI) of 33.8. Orbera balloon implanted in July 2015 and withdrawn in January 2016. At the end of treatment, the patient presented final weight of 69 kg, BMI of 23.3, with loss of 31 kg or 31% of BWL%. Until April 2017, the patient was 74 kg, with maintenance of 83.8% of the weight lost in a follow-up of 15 months.
—Case 2: ERM patient, female, 42 years old, with inicial weight of 96 Kgs and BMI of 36.1. Orbera balloon implanted in September 2014 and withdrawn in March 2015. At the end of the treatment, patient had a final weight of 66 Kgs, BMI of 24.8, with loss of 30 Kgs or 31.25% of BWL%. Until April 2017, the patient was 70 kg, with maintenance of 86.6% of the weight lost in a follow-up of 25 months.
—The weight loss reported in these two cases, as well as the long-term maintenance, do not reflect the means expected with the BIG treatment. On the other hand, these cases exemplify that better results are possible because this treatment depends on the patient's involvement, effort and dedication to follow the guidelines of a multidisciplinary team (doctor, nutritionist, weight loss coach and psychologist). The greatest benefit of treatment with IGB is not only weight loss, but the possibility of serving as a tool to assist a profound and true change in lifestyle, which when achieved can bring benefits and allow the maintenance of long-term results.
