 Role of ketamine in The breakthrough pain management after neuro axial narcotic blocks for postoperative pain control Category Details Scientific abstract: Chronic pain Role of ketamine in The breakthrough pain management afer neuro axial narcotic blocks for postoperative pain control 4861 ,Breakthrough pain management in neuro axial narcotic blocks is I complex problem. adequate pain management vs respiratory complications has to be carefully weighed against each other narcotics and And NSAIDs are the options for this management we ha e tried ketamine for this breakthrough pain management as has less respiratory events and side efects profile we have selected 30 patients each into two categories for this study All 60 patients received intrathecal duramorph 300 mics for postoperative pain control 30 of them managed with intermittent morphine push supplemented with Tylenol infusion for breakthrough pain management The other 30 patients received ketamine 50 mg infusion over 30 minutes q 6 hrs prn for the breakthrough pain management the requirements and pain scores were compiled and compared from the both groups we found ketamine Group has superior pain control with no adverse efects. pain scores were 0 to 3 in the ketamine Group and pain scores were 4 to 5 in morphine groupDiscussion  the average infusion requirements for ketamine Group is 0 to 1 total of 12 injections required for the whole 30 Group. morphine Group required about 90 boluses in total for the whole 30 Group which is about 3 per pt. side efects profile is significantly lower in ketamine Group . Nausea, pruritis incidence about 2% and respiratory events were zero in ketamine Group morphine Group the nausea , pruritis being about 25% and respiratory events were about 4% . Ketamine has been found to be superior in management of breakthrough pain management for neuro axial narcotic blocks. it has minimal side efects profile and efective pain control
