We present a case of a 31-year-old female that had chronic pain and was on Methadone therapy. She developed signs and symptoms of Opioid Induced Hyperalgesia and was offered Mexiltene therapy. She responded to this treatment regimen with subjective improvement in NRS pain scores and functional improvement in her daily activities and sleep. This is the first report of its kind in which Mexiletine therapy was successful in ameliorating the symptoms of Opioid-induced Hyperalgesia.
